How to get a discharge letter from my current clinic?
To request a discharge letter, you’ll need to contact your current clinic directly. Most clinics require you to submit a written request or complete their transfer of care form. The discharge letter will outline your treatment history and is usually sent either to you or directly to your new healthcare provider.
At Healing Leaves Clinic, we recommend asking your current clinic for:
- A copy of your discharge letter
- A copy of your health summary or My Health Record update
This ensures your new Doctor has the information needed to provide safe, continuous care.

Do I need a health summary?
No, a health summary isn’t required, but we do recommend providing one. Having access to your health summary helps our doctors understand your medical background more accurately, which can make the consultation smoother and may speed up the prescribing process.
What does it cost?
We’re privately billed with clear, upfront pricing. Choose the consult that suits you.
How do I get my medication?
You’ll receive an eScript via text message on your mobile. Collect in‑store from any pharmacy nationwide, or have it delivered to your door.
How quickly can I see a doctor?
Usually same day or within a week, subject to availability. After your nurse screening, we’ll handle the doctor booking for you.
Do I need a referral?
No referral is required. If you have one, feel free to upload it at booking.
How do I know if I’m eligible?
We start with a quick 15‑minute nurse screening to review your health history and current needs. If clinically suitable, we’ll book you with the first available Doctor.
